Clinical Care Coordinator (Grade 5)

We have a rewarding position at Doutta Galla Harmony Village Aged Care in Shepparton for a Clinical Care Coordinator. This position offers an exciting opportunity to work in a role and with an organisation you can be proud of.

The role is full-time: Monday to Friday 0700-1506 and is responsible for the delivery of clinical care services including care planning, review and assessments. 

What you will bring:

• A minimum 5 years experience as a Registered Nurse with Aged Care experience.
• Dedication to resident care and support
• Excellent clinical knowledge regarding medical conditions, palliative care, and care of people living with dementia including managing challenging behaviours
• High level interpersonal and written skills
• Experience managing staff and performance management
• Ability to effectively communicate with residents, resident representatives and other health professionals
• A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical care
• Current AHPRA Registered Nurse DIV 1 registration with no conditions
