An exciting new full time (Mon to Fri 0900-1706) opportunity to work alongside a highly dedicated team to deliver outstanding care to residents across Doutta Galla's eight aged care homes.
With a reporting line to the Executive Manager Clinical Governance, the Clinical Nurse Specialist Wound, Ostomy, Incontinence is responsible for ensuring care is of the highest quality for the older person dealing with a variety of chronic and acute skin integrity issues including wounds, ostomies and / or incontinence issues, as well as more complex conditions, including those living with diabetes.
This role includes:
- The management of complex skin integrity issues including but not limited to: pressure injuries, chronic and acute ulcers, burns, Incontinence Associated Dermatitis (IADs), diabetic foot wounds and skin tears
- Assessing the older person’s skin, assessing, cleaning and monitoring wounds, and focussing on skin injury prevention strategies
- Providing education on injury prevention, wound care, skin tears, infection prevention and control, antimicrobial stewardship, pain management, and pressure injury prevention, staging and care
- Creating and completing wound care charts and plans to expedite wound healing and avoid skin breakdown
- Working closely with residents, members of the multi-disciplinary team and substitute decision makers to ensure personalized treatment plans are followed correctly in order to prevent future infection or injury
- Ensuring clinical care aligns with evidence-based best practice and agreed and documented clinical guidelines, pathways and standards.
- Demonstrating and promoting clinical expertise and best practice standards.
- Monitoring the quality of care, identifying areas for improvement and supporting staff to deliver care of the highest standard.
To be successful in the role you will have:
- In depth experience in assessing and treating the older person with complex wounds, ostomies, or incontinence conditions.
- Demonstrated experience in clinical leadership including using effective communication and interpersonal skills.
- Ability to interpret aged care and disability standards and develop and/or implement related policies and procedures.
- Ability to assess, review results, and implement necessary improvements to ensure best practice is achieved
- Demonstrated experience in clinical leadership including using effective communication and interpersonal skills
- Strong clinical governance knowledge and current knowledge on best practice
- Excellent written and verbal communication skills.
- A Graduate Certificate in Stomal therapy nursing and / or Diabetes Education and / or Wound Care is preferred
